for dealing with employees' personal problems that can affect their work
situations. It is also an aid to employees and their family members who
voluntarily use the program as a means of resolving personal problems.
Any employee receiving help under this program will be given careful
consideration and confidential assistance.

receive up to three (3) working days per occurrence for the death of a

Employees are immediately eligible for injury leave and receive salary
continuation for up to 60 days for an on-the-job injury. Injury leave is
reduced by the amount paid under Worker's Compensation.

Any full-time employee who is summoned to serve on a jury or appear as a
witness on behalf of the City may be permitted to be absent from work
with pay by the department head for the time actually required by such
duty or appearance.

Legal Plans USA is a voluntary legal benefits program. The plan offers
free legal consultations, free legal correspondence, and reduced rates
for other legal services. The bi-weekly rate is $4.61.

Employee: Basic Life ($10,000)
Eligible after working one full pay period. $10,000 term life and equal
AD&D provided to each eligible employee paid by the City of Lubbock.
Optional Life (1X, 2X, or 3X)
An amount equal to 100%, 200% or 300% of the employee's annual earnings
(rounded to the next highest $1,000 of benefit) to a maximum benefit of
the lesser of $500,000. Rate is based on salary and age.
Dependent Life
100% of employee's coverage in $5,000 increments up to $50,000 (if
participating in optional life).

Two choices are offered.
1. After being continuously disabled for at
least 180 calendar days, an employee may become eligible to receive
sixty percent (60%) of their base pay.
2. After being continuously
disabled for at least 90 calendar days, an employee may become eligible
to receive sixty-seven percent (67%) of their base bay. For more

In a municipality with a population of 10,000 or more, each member of
the fire or police department is entitled to receive, in addition to all
other money paid for services rendered in the department, longevity pay
of $4 a month for each year of service in the department, not to exceed
25 years.

who are members of reserve units or National Guard receive fifteen (15)
days paid leave to attend annual training.

Employees may select coverage at 1x, 2x or 3x their annual salary.

Premium Only Plan - Allows the employee to pay premiums for certain
optional insurance premiums on a pre-tax basis. Requires annual election
and has some restrictions.
Medical Spending/Dependent Care Accounts - Allows the employee to pay
for certain un-reimbursed health expenses and dependent care expenses on
a pre-tax basis.

Employees in full-time positions are eligible for sick leave with full
pay for fifteen (15) working days per year. Sick leave is provided for
employees too ill to perform duties. Up to 90 days unused sick leave is
paid to employees in good standing upon separation with 15 years
service.

The stability payment is made at the end of each calendar year for
actively employed regular full-time employees who have completed three
(3) or more calendar years of continuous service. Stability pay is
$150.00 the first year eligible, plus $50.00 for each additional year to
a maximum of 24 years of service with a maximum of $1,200.00.

The City of Lubbock offers a retirement plan through the Texas Municipal
Retirement System. The City of Lubbock's TMRS plan requires a
contribution (made by means of bi-weekly payroll deductions) equal to
seven percent (7%) of the salary of each regular full-time City
employee. The City matches employee contributions on a 2 to 1 basis.

Accrued vacation benefits may be taken on a prorated basis after January
1 of the first year. Full-time employees earn vacation leave at a rate
of 10 days of vacation for one (1) full year of employment. Employees
accrue an additional day for each year of service over five (5) years,
maximum is 20 days. The additional day of vacation will be available
after 6 calendar years of service.
